- Your full name

- Your phone number

- Preferred dates and times for the booking

- The city of our meeting

I also require ONE of the following:

- A reference from another independent provider whom you have seen in the past six months.

- A photo of a government delivered ID (passport or driver's license).

 

Please note that I do not accept any meeting without proper screening, and that a 50% deposit may be required for new suitors to set up a date.
